To help ensure safe and effective care:
- Patients must provide accurate and up-to-date medical information
- Medications should only be taken as prescribed by your provider
- Follow-up appointments are required to monitor progress, effectiveness, and side effects
- Patients should notify our office of any medication reactions, hospitalizations, or major health changes
- Controlled substances may require additional monitoring, including treatment agreements, pharmacy verification, or periodic evaluations
- For patient safety, providers may recommend in-person visits when clinically appropriate
- Emergency or urgent mental health concerns should be directed to 911 or the nearest emergency room
Medication decisions are made at the discretion of the treating provider based on clinical evaluation, medical history, and safety considerations.

To help us provide timely care for all patients, we kindly ask that appointments be managed responsibly.
- Intake forms must be completed prior to appointments
- Payment is due on or before the day of your appointment
- Appointments canceled with less than 24 hours’ notice may be considered a no-show
- Missed appointments or late cancellations may be subject to a no-show fee
- Repeated missed appointments may result in limitations on future scheduling
- Insurance coverage does not guarantee payment; patients remain responsible for applicable copays, deductibles, coinsurance, or denied claims
- Patients are responsible for notifying our office of insurance changes or inactive coverage
- Insurance verification may take up to 72 business hours
- Medication refills require active follow-up care
- Refills are not guaranteed without a scheduled appointment
- Controlled substances are not replaced if lost, stolen, or filled early
- Providers may require periodic check-ins before continuing treatment
Insurance and Service Fees
We accept most major insurance plans. Coverage and out-of-pocket costs vary depending on your individual insurance benefits.
Patients may still be responsible for:
- Copays
- Deductibles
- Coinsurance
- Non-covered services

Telehealth Information
Most appointments are conducted virtually through a secure telehealth platform unless otherwise directed by your provider.
For telehealth appointments:
- Patients should attend visits from a quiet and private location
- A stable internet connection and working camera/audio are recommended
- Patients must be physically located in a state where our providers are licensed at the time of the appointment
- In-person evaluations may be required at provider discretion
Communication Policy
- Our office strives to respond to messages within normal business hours
- Prescription refill requests may require 2–3 business days for review
- Messages sent after hours, weekends, or holidays may receive delayed responses
- Electronic communication should not be used for emergencies
- Patients experiencing a medical or psychiatric emergency should call 911 or go to the nearest emergency room
By communicating electronically with Happy Sol Health, patients acknowledge and accept the potential limitations and risks associated with electronic communication.
Controlled Substance Policy
To support safe prescribing practices:
- Patients may be asked to use one designated pharmacy
- Early refills are generally not provided
- Lost or stolen medications may not be replaced
- Follow-up appointments are required for ongoing prescriptions
- Additional monitoring or documentation may be required based on provider discretion
- Medication decisions are based on clinical judgment, patient safety, and applicable regulations

- If using insurance, our team will first verify your benefits and eligibility prior to scheduling whenever possible.
- Patients will be asked to provide a copy of their insurance card and identification.

Complete Intake Forms
- Prior to scheduling and your appointment, intake forms and required documentation must be completed.
- This will include identification, insurance information, consent forms, and medical history.
Schedule Your Appointment
- Once insurance verification and intake forms are completed, our team will help schedule your appointment. We offer convenient telehealth appointments for eligible patients.
Meet With Your Provider
- During your initial consultation, your provider will review your concerns, symptoms, medical history, and treatment goals.
- Personalized treatment plan, which may include medication management, therapy referrals, lifestyle recommendations, additional evaluations, and ongoing follow-up care to help monitor progress and adjust treatment when needed.
